What Labs Really Matter in Hormone Therapy — and Which Ones Don’t
Hormone Replacement Therapy (HRT) has transformed countless lives — relieving hot flashes, improving sleep, lifting mood, and restoring energy and libido. Yet many patients are confused, frustrated, or misled about the role of lab testing in hormone therapy. Questions like “Should I test saliva or blood?”, “Does a normal lab rule out imbalance?”, or “Why do my symptoms persist despite normal labs?” are extremely common.
Unfortunately, many lab tests sold directly to consumers or used by clinics aren’t clinically reliable or useful for guiding therapy — and may even mislead patients. On the other hand, certain tests do provide meaningful information when interpreted by experienced clinicians.

📍 Sex Hormone Binding Globulin (SHBG):
SHBG impacts the bioavailability of testosterone and estradiol. Knowing SHBG levels helps interpret total versus free hormones more accurately.
📍 Monitoring Testosterone Dosing:
In both men and women, serum testosterone levels generally track dose — making it a reliable way to monitor and adjust therapy.
📍 Established Reference Ranges:
Blood tests have well-documented norms and quality standards, which clinicians rely on for interpretation.
Important Limitations
❗ For Sex Steroids, Total Hormone ≠ Bioavailable Hormone:
Most serum tests measure “total” hormone (bound + unbound). Only the unbound “free” hormone is biologically active, but free hormone assays are not always available for all sex steroids.
❗ Snapshot in Time:
Serum values can vary throughout the day or cycle, especially in perimenopause. A single value reflects only one moment — not overall hormone dynamics.

3. Saliva Testing — The Controversial “Bioavailable” Marker
Saliva tests are widely marketed — especially in functional medicine and direct-to-consumer hormone kits — as measuring free, bioavailable hormones that actually reach tissues.
What Saliva Testing Measures
Saliva assays sample the unbound fraction of steroid hormones (e.g., estrogen, progesterone, testosterone), which theoretically reflects the biologically active portion.
Why That Sounds Good — But Isn’t Proven
Despite its appeal, major medical societies do not recommend saliva testing for hormone therapy management — and for good reasons:
🔹 Lack of Standardization: Salivary hormone assays aren’t standardized, lack quality control, and show high variability between labs.
🔹 Poor Reliability for Sex Steroids: Evidence shows poor correlation between salivary and serum levels for several hormones, including testosterone, especially in clinical settings such as HRT monitoring.
🔹 FDA & Professional Warnings: No salivary or urine hormone tests are FDA-approved for steroid hormone measurement, and organizations like the Endocrine Society and North American Menopause Society (NAMS) warn they are unreliable for adjusting therapy.
One clinical guideline explicitly states that salivary (and urinary) hormone testing is not recommended for adjusting hormone levels in therapy due to unreliable and variable results.
When Saliva Can Be Useful
There are niche applications — for example, diurnal cortisol patterns in stress assessment — but this is not the same as using saliva to tailor sex hormone dosing.
👉 Bottom line: Saliva testing can seem attractive because it’s non-invasive. But for sex hormones in HRT, it does not provide reliable data for dose decisions and has significant methodological limitations.
4. Urine Testing — Metabolites vs. Hormones
Urine tests are often used in research or for specialized purposes — such as metabolic mapping — but they are not direct measures of circulating hormone levels.
What Urine Tests Reflect
Urine can measure hormone metabolites — how the body is processing and breaking down hormones after they’ve circulated through tissues.
This can be useful when:

❌ “Comprehensive” Urine Hormone Panels
